Beginning for the 2008-2009 academic year, the MBU Sports Medicine Department
has started a new system for medical forms. We are no longer mailing out
the forms that student-athletes need to complete. These forms are now available
for the student-athletes online.
The following forms are available for download. If for some reason you cannot
access these forms please check to see if you have Acrobat Reader on your
computer. If you do not, you can download Acrobat from this page. If you
still cannot obtain the form they can be obtained by emailing or calling
the Sports Medicine Department at dillm@mobap.edu or 314-392-2399
All incoming freshman and transfer student-athletes must complete the entire
New & Transfer Medical Information Packet and have a physical examination
prior to practice, competition, and participation. The physical examination
will be administered by our team physician when his/her team is assigned.
Returning student-athletes must complete the entire Returning Athlete Medical
Information Packet and attend a physical when his/her team is assigned.
Dates and times for each sport’s physical are posted under Physical
Examination Dates & Times. (Can you make the highlighted part a link

Our athletic physical examination will be conducted by our team physician
to clear the athlete for intercollegiate athletics. A physical exam by a
personal physician will not be accepted. All medical information must be
received by the Athletic Training Department no later than July 15th, 2008.
